Costa Rica’s philosophy is that everyone in the country is entitled to receive health care, regardless of status or age. That premise is fulfilled through the social security system, Caja Costarricense de Seguro Social (referred to as the Caja). All Costarricenses are eligible to participate, as well as foreigners.

Caja health care facilities are located throughout the country and participants are assigned to a treatment center according to their place of residence. All individuals may join and receive benefits, without restrictions for pre-existing conditions. Therefore, the Caja is especially beneficial for persons with a history of chronic illness, which requires ongoing medication or recurring difficulties such as orthopedic problems.

Individuals may apply for health care through the Caja individually at the headquarters in downtown San Jose (across from the National Theater), or through ARCR’s group policy. Premiums for individuals are based on their declared monthly income (times 11%) and ARCR’s rates are priced yearly on a group basis.

Once you have paid, to register at your clinic you must provide the registration personnel with proof of residence (a letter from your local police), citizenship or legal residence, and payment. You will then be issued an identification card to receive services from that clinic. When you first request services you will be assigned a doctor who will be your primary care provider.

Your doctor will determine what treatment is necessary and whether or not your condition warrants referral to a specialist. Specialists are seen by appointment, scheduled in advance, after referral by your primary care provider. Follow-up visits are also scheduled in advance. However, regular visits (such as when you have the flu) are assigned on a first come, first serve basis. That means when you want to receive treatment you must be there when the clinic opens, wait in line, present your identification card, and you will then be given an appointment.

It also includes diagnostic procedures, laboratory testing and prescription drugs. At most clinics, there is a pharmacy on-site and prescriptions are usually filled within a couple of hours--or you may choose to return the next day to pick them up.

Overall, Caja professionals are conscientious, treatment is satisfactory and the cost affordable.
